Field names

Fields are listed by the names the form's author gave them, which are usually close to the printed labels. The numbers on the preview show which box each one fills.

Checkboxes and radio buttons

Why does it say my PDF has no fields?

The form is probably a scan or a flat printout without fillable fields, or an XFA form made with old Adobe software. For a flat form, add text on top with a PDF editor.

Should I flatten the form?

Flatten it when you send the final version, so nobody can change your answers. Keep an unflattened copy if you might need to edit it.

Can I sign it too?

Yes: save the filled form, then open it in the PDF signer to add your signature.
